How does the urinal tablet holder thing get turned upside down every once in a while? It has to be the cleaning people because nobody else would touch it, but they're pretty careful about cleaning so wouldn't they place it correctly? I don't know.....

I doubt anyone is powerful enough to flip the tablet holder over. The good thing about it being upside down is that it blocks the "splash" from hitting you.
